Use this form to ask our office to review additional creditors that were not included in your bankruptcy paperwork. Please complete all information as accurately as possible. Submitting this request does not mean an amendment has already been prepared or filed.

Amendment Fee Disclosure

The fee for preparing and filing one amendment to add creditors after your bankruptcy case has been filed is $250.00. By submitting this request, you understand that this fee applies to this amendment request. If additional creditors need to be added later through another amendment, an additional fee may apply.

Important: Creditor Address Requirement

You must provide the best and most complete mailing address you know for each creditor. Bankruptcy notice must be properly mailed to the creditor. If a creditor is not properly listed or does not receive notice at a valid address, that debt may not be discharged. Returned or undeliverable mail may affect whether the creditor received proper notice.
